Central Grampians Palliative Care: Enhancing your quality of life

Central Grampians Palliative Care is a community based service working out of the EGHS Community Centre.
The service offers health care to patients with a life threatening illness and supports both the patient and their carers. Health care offered is by trained nurses who care for the complete needs of a person, physical, emotional, spiritual and social.
The service is free and funding covers lending of equipment, home care support, and assisting with aids such as handrails and ramps. The focus is to offer a service that will enhance a person's dignity and maximise quality of life.
An integral part of the service is care co-ordination which involves liaising with the patient's GP, allied health staff such as Occupational Therapy, District Nursing, Physiotherapy, Dietician, Speech Pathology, and a Palliative Care Physician who visits regularly from Ballarat. Other agencies that Palliative Care refers to include Carer's Respite, Council home based services including home care and home help and home handyman.
Central Grampians Palliative Care covers the three municipal areas of Ararat Rural City, Northern Grampians Shire and the Pyrenees Shire and employs one full time and two part time nursing staff, and a part time administration in addition to extra administration from East Grampians Health service.
